I could be wrong but I think Japan DID paint him as actually being gay, while in the English localization, he's just confused. Ultimately, I think him being confused works better for a more complex character arc because it's not nearly as straightforward as him ending up gay. , at least when it comes to the way it is handled in P4. In Kanji's case, the struggle of him being gay boils down to little more than "NOOOOO I DON'T WANT PEOPLE TO KNOW I'M GAY!" but with him being confused, it results in him questioning his identity as a whole due to the social pressures of acting masculine. He's straight but his attraction to Naoto makes him question himself because at the time, he still thinks she's a boy. Not to mention his hobbies and interests are more feminine which makes society question him and it piles onto his still developing sexuality which leads him to question himself. That uncertainty is mirrored in his dungeon with everything being gay as fuck because he himself is caving to what society thinks of him and struggling with his more feminine aspects despite being a straight male.

It's a much more interesting arc for him to be straight in the end because it leans harder on social stigmas and pressures (something which is a huge focus of Persona games) and plays counter to stereotypes, rather than into them, which is what happens if Kanji actually DOES turn out to be gay. Not to mention, nobody in the game actually cares if he were gay (save for Yosuke but only because he's an idiot and thinks everything Kanji does is to hit on him) and they even say that to his face so it waters down the conflict and makes the entire arc a non-issue.
