I just tried that 3 times. First time I couldn't infect Madagascar and Argentina.
Second time was Madagascar only.
Third time was Argentina only.

even if it doesn't work in-game due to mechanics that's still the logical way if you were trying to make a disease to wipe out all humans. If your disease immediately makes people start bleeding from every orifice but isn't super contagious you would logically be fucked.
