ALL forsaken are slowly rotting. The only ones with any kind of rot protection are the Lich King and his death knights.
It's canon that most forsaken need to swap out old parts for fresh ones when they no longer function, and one of the biggest fear for Forsaken is when their brain
Necromancy significantly slows down putrefaction, but it doesn't stop it completely.

And Sylvanas covering up more, despite the meta reasons, is being handwaved as Sylvanas being a vain creature who can't stand looking at herself slowly starting to rot in the mirror.
Part of the reason Sylvanas is so obsessed with finding true immortality for herself is because she knows that even in her long-lived Forsaken body, it's eventually going to start falling to pieces. If her body wasn't slowly deteriorating, she wouldn't need to find another avenue.
