When it's in that weird spot of being extremely gratuitous just for the sake of it, but not to the point where it's intentionally near farcical like Bulletstorm or Mortal Kombat.
There's a point where the game, despite trying to be grounded, almost seems to revel in extreme violence and death. I hate to compare video games with movies, but good serious movies use that sort of violence sparingly as punctuation, usually to shock the viewer. There are games where you have enemies begging for their lives, crawling around mortally wounded, or just agonizingly bleeding out, and I just wonder how that's supposed to be fun. At best it's disturbing and anachronistic, at worst you just become desensitized to it and don't even care, so what is the point?
I haven't played The Last of Us, but I'm pretty sure killing in that game is supposed to be disturbing (though I'm sure that feeling fades really quickly). But then you have a game like Uncharted where it's supposed to be a fun romp and it glosses over the fact that you're murdering hundreds of people without blinking, yet shows their heads getting realistically blown apart.

The best amount of gore in any game has to be RE2, where it's visceral and robust but also serves a gameplay purpose. It's the right amount of it and any more than that (which is already a lot) turns into gratuitous violence and thats not worth it. Of course this also scales with realism. Gibbing in like TF2 or HL1 is so unrealistic that it barely registers as gore.
The wrong amount is like the amount in mortal kombat 11 where it's just everywhere and non stop and is also meaningless beyond shock value. That is the definition of gratuitous violence and it sucks ass.

No such thing as too gory. Hyper-realism makes things feel more visceral, over the top is funny and campy, and everything else in between just makes it feel like the violence performed in the game has more impact to it. For instance, I love the ultraviolence in Hotline Miami, and while not necessarily being high fidelity the context of the game still makes it feel intense when you pour the boiling water onto somebody's face or stick a drill through their temple.
What's more is in games like Red Orchestra 2 and Rising Storm the cries and death rattles of the dying adds even more immersion to the point where it almost feels gross. The kills with the flamethrowers always sound so fucking painful. Killing Floor 2 has wonderful amounts of viscera and gore. Tripwire in general does a fair job doing that.
