I figured out what it is. Every feature is drawn well enough, but the facial proportions are a bit uncanny once you notice it. Like post-timeskip One Piece, all the facial features are crammed on the bottom half of the head. Look how much of the head is forehead and how the eyes line up with the ears wrong. Here it looks okay at first, but there are other drawings where its more obvious.
