Stories aren't built around characters acting like normal people. Even ones striving for realism are typically built around the cast being exceptional in some regards in order to give you reason to be compelled by them.
And in any case, if the argument is that everyone should act normal, Fuutarou's a teenaged boy who knows he has multiple options and understands that picking ANY of them will complicate his lifestyle. It's not uncommon for normal people to decline relationships for no reason at all except that they aren't feeling it, and in this case, it's a girl who'd harassed him for half a year before suddenly turning around and one that he's always avoided dealing with.

When you take into account the Yotsuba reveal being right after Miku makes peace with Ichika and Nino the timing begins to make sense, only she and fat are struggling.

Miku is really good in some arcs and much less so in others, but the erratic characterization from arc to arc leads to a product that's lesser than the sum of its parts.

I'd disagree here, however. Yotsuba's characterization was built up throughout the whole manga but we just lacked the context at the time. With the context we have now, she's very clearly a well defined character. She's also probably carrying the most mental baggage of any of the quints. I like her a lot, but I think she needs to come to terms with her psychological holdups, and considering how she's still disregarding her physical and mental health as shown recently, I think it's inevitable that her arc will take her on the road to saving herself. Now my thinking is that on the one hand, it wouldn't be out of left field at all if Negi made her the bride, but on the other hand, since she's so tied down by the trauma of her past and the impression Shoutarou left on her, it may be that the healthiest outcome for her would be to let it go. Personally I'd only really accept a Yotsuba ending if she drops the invincible schtick she started when she met Fuutarou as lolikano and stops being afraid of showing her weaknesses, insecurities, and darker emotions.

I think Itsuki does have a pretty well defined character; she has a clear moral code, clear ideals, and clear goals in mind all defined by her upbringing, and she's pretty cute to boot. At this point in the story however, considering that she still hasn't entered the bowl, I personally don't feel like any of the ways that she could be realistically written into the bowl would be very satisfying. If anyone has any suggestions here, let me know and I may change my opinion.
