I think it’s most likely that in the snapshot of future memories that Eren received, he saw Zeke as a part of those. It’s also possible that since it’s been established in the narrative that blood relationships are a factor in how memories work, Eren made the determination that way.
Eren’s thank you sounds very much like Zeke specifically made it possible. It makes no sense that Historia would be able to reach Grisha the way Zeke was able to.

A side point but still interesting is that in chapter 63 that when Eren and Historia touched, Historia received memories of Frieda while Eren received them of Grisha. There was no crossover. Historia had no idea about Frieda’s fate or what memories Eren received. I think it lends credibility to how memory interactions best work.
