This is the fault of shitty writing. Mangaka don't know how to manage stakes and just escalate constantly. What makes things suspenseful is your investment into the characters and consistency within the world to the consequences of either bad luck or poor decisions.
The moment the losing of a fight doesn't result in the logical outcome you lose a good portion of your audience's suspension of disbelief.
It's like a fiancee who wants a wedding day every third "chapter." You get one, that's what makes it special.
