I always hate using meme words but this is pretty accurate. Ultimately what people mean by that is that the tone and nature of the work mismatched so hard they couldn't take what was going on seriously which can be the point in some works. It's actually pretty hard to find something that was so depraved no human has done it or would do it if given a chance. But it is possible to take those actions and play them in a way that doesn't match how the setting has built itself and causes you to break out of engagement of the work.
