I'd be fine with it mostly - even with the same ages. I don't see why a slightly younger character can't care for a slightly older one. It would even lead to some interesting dynamics as the older sibling protects out of familial responsibility while the younger protects because the older was hurt.

What might have to be changed is stuff havign to do with Tanjirou's scar, hair, and inheritance from his father - things that wouldn't work as well on a demon and are needed by the slayer. I see they gave Nezuko the earrings, but other elements would have to be adapted.

The empathetic act from Nezuko as MC would be a lot more bearable. It just doesn't sit right for a male shonen MC to act the way Tanjiro does.

Just because you're incapable of empathy doesn't mean every guy is.

Many classic shonen protagonists had manly qualities and it's what made them great. Trying to stand out by making your MC the opposite of those classic protagonists is just awful and all you've done was write a female and made her a boy because the series wouldn't sell otherwise.

How about if they were able to swap roles somewhat freely? For example, them being siblings allows each to carry the burden of demonic blood for a time before swapping to the other, and failure to swap in time leads to permanent demonization.

Invert more elements of the show.
Instead of killing every demon and searching for a cure for her brother, she kills her brother in the first episode, and spends the rest of the series curing every demon she finds sailor moon style.

Every time the demon of the week almost defeats her, a mysterious man wearing a blindfold shows up and gives her a heroic speech that helps her win, and the guy turns out to be Tanjiro.

He doesn’t even need to have a "serious" crying face, he can just cry.
He is the "first son" after his father passed away he effectively replaced him within a large family becoming a responsible working adult extremely fast.
Tanjirou easily trash talks both pillars who disapprove of Nezuko even at the later stages, he trash talks anyone who dislikes Nezuko.
Because he can smell out emotions he often acts in patronizing ways to someone less experienced or confused with ones emotions, primary reason probably lies in his initial loss and the blame he took for it.

I'm not sure if this would "work" as a story set in this universe. If only because I don't really know what Nezuko was like before she transformed. But as a premise? Sure, cute demon hunter girl traveling around with cute demon brother sounds like something I would enjoy.
