What’s worse is the uselessness of the premise can be easily fixed in a way that can make the plot more interesting. For example, other stuff could bleed over between worlds, so the MC might have a leg up on others because he can recognize the stuff that shows up, but it can potentially be used against him.

"muh limits" is unironically shit writing. There's no such thing as hard limit in reality, only diminishing returns until you die of old age. In fiction the concept of hard limits exists only to excuse the fact that you don't have to write any more development for a certain character. You can just say that he's hit his human racial limit at level 40 or some shit and sidestep any attempts to overcome humanity to get more powerful. Without hard limits you have to deal with the numerous ways to bullshit yourself out of soft limits with magical augmentation or whatever. You can also not have to deal with the logical conclusion that 5000 year old fat ugly bastard would be ungodly talented at everything with muh game mechanics, he just hit his cap at age 60 and stopped growing after that.
Level caps and game mechanics only exist to provide easy ass-pulls and cop-outs for amateur writers. Change my mind.

Things slip away as people age, and I'm of the opinion that anyone who managed to live for 5000 years would have forgotten 90% of what he'd learned. There's also the matter of an aging body degrading physical power, but then we get into the mechanics of immortality.
Basically, the problem of soft limits is that they're all of different kinds. People have limits, but if you want to address them in fantasy you have to do so individually. It's easier to handwave them (i.e. Tolkien's perfect elves who never age or forget) or to impose broad-spectrum hard limits as a model.
Also, this is just a nitpick, but in your model a 5000-year old would be ungodly skilled, not ungodly talented. Talent is the potential you're born with, skill is acquired ability.

i want a fucking Isekai where it actually explores how fucked up it is that you're transported away to another world. The MCs are never seeing their friends and family ever again, that has to be traumatizing and depressing, or on the flip side, it has to be a blessing in disguise because its a second chance for them since they finally get to escape their horrible lives. That sort of conflict would be really interesting to explore but for some reason, it never is.
The average isekai fan has no friends and is hated by their family so that wouldn't be marketable.

Soft Limits actually allow for writers to write better characters that learn and develop new ways to circumvent their soft limit (power of friendship, mechanical advantage, new strategies, anything really) and keep growing and develop.

A similar idea is when in time travel isekai, the MC is knowledgeable of the time period and uses it influence events. For example, the MC teaching the three-volley shot or building a fort overnight in Oda Nobuna.

Isekai stories tends to be so easy that there is no tension to read. Its hard to find one where the MC doesn't start dead, won't become OP fast and doesn't go into a fuckfest of a story in less than 10 volumes worth of story.

If you want to set it forward without going into our modern era, maybe set it in a Victorian era where industrial revolution era machines are well under regular use. We could set it in a world set in the far future, where the protag has to figure out how to operate a spaceship, navigate strange worlds, and actually have to negotiate with new technology.

The problem is 18th century court intrigue/society stuff like Dangerous Liaisons is all about the intricacies of high society and people cleverly navigating them. I feel like that could make it more interesting. Sure, in a might-makes-right dark ages world the isekai MC with cheat powers gets everything his own way, but what if he's in a world where you need social grace to get ahead? He's a loser who can only solve things with violence, but violence can only get you so far; he's going to find himself getting manipulated, fucked over, used as a weapon, and generally having all kinds of strife in his experience. You know, actual story tension.
The power balance would be important, of course. He should be given a power on reincarnating that's strong enough to get him cocky about being an isekai protagonist, but not "I can wipe out armies" strong, like some are. He'll still get killed if he makes too many enemies.

That's basically what happened to the MC in Re:Zero. MC has the power to essentially restart from a save point every time he dies and thinks it makes him unbeatable. He eventually learns he's a joke in the iseaki world who's been played for a chump by all of the major factions because he's such a NEET loser.
I think your idea is workable, though. It's essentially the equivalent of stories about a country bumpkin or someone from a shit background somehow finding themselves thrust into high society.
