Youre confusing two different scenes. The one with the gun is where he shows misato whats in terminal dogma. Whn he dies misato was (probably) at seele but ended up knowing what kaji did and therefor anticipated that he would be killed. As to tho did it it was likely some random agent, why send a scientist or the woman in charge of the kids in olace of a trained specialist? Makes no sense

Kaji only appears in a memory of Misato's in EoE, I'm inclined to think he's not there. He's in EoTV and part of the dialogue, so maybe, or it's someone's impression of him and what they think he'd say because it's fucky like that. Manga adds a scene that looks just like how Rei reaps people. If this is Rei taking the image Kaji is most comfortable with, though, none of them are smiling and it just makes me think Kaji is self-hating enough that what he most wants is to be punished by the street family he secondhand murdered.
