So many times I'll be reading an isekai in some medieval setting and they fucking pull out a ballpoint pen, a radio, mayonnaise, a modern bath or some such shit.

I've recently gotten into Kuro no Maou. It's one of my personal favorite and I've read over a two dozen isekai. But I hear the translations are shit so I wouldn't recommend it unless you can read jap.
Generally isekai written before 2012-13 are better than those who came after.
I just got caught up on Shinju no Nectar and somehow a breast sucking fetish story is the best isekai I've ever read. It avoids all of the usual isekai problems that makes so much of the genre shit and uses the premise enough that there's a good reason it's not just straight fantasy.
