All demons come from Muzan and are former humans, including the spiders. The only exception is the doctor demon's buttboy who she made through her own effort but was never able to repeat the effect and convert anyone else. The one spider demon just has an obsession with family so he took several other weaker demons and modified them to be more like himself. There are no examples of a demon giving birth or having children in the manga so we don't know if its possible yet.
