About Miko, Kobachi is her best friend, so at least she cares about Miko and wants the best for her, but Miko's situation is more complex than Kaguya because not only she has personality problems like Kaguya, Ishigami likes Tsubame, not her. I think we all get a little wrapped up in our head cannons and whatnot when thinking of the series. I don't think that the early pairing of Ishigami and Hayasaka or with Fujiwara were too ridiculous at the time, although in hindsight it isn't that off base seeing the current arc shipping Miko. However, many people were eager to lump the Miko thing in with the other things mentioned.
Onodera was one of the few people that Ishigami didn't blot the face out during his normie arc. She also wasn't exactly friendly with him until it became obvious that he was working towards self improvement. As we all know everyone in the cheer squad accepted him and he regularly interacts with the two girls.
I think everyone puts his pariah status in the back of their mind when discussing Ishigami related issues. He games and is an otaku but he used to get good grades and was athletic before the incident. Others around him that know he's a great guy are probably worried he'll shut down again, which he almost did when he wished for everything to end while falling. He's "our guy" and that includes the depression and anti-social tendencies that used to overwhelm him.
So while obviously Onodera is actually snooping and Miko is tsun about her dere, there's also the "if japan had guns Ishigami would be a school shooter" way people see him and the fact that he doesn't care about how most people see him in mind whenever you deal with him.

Which is to say, she was literally listed as the "Flip-side Heroine" to his "Flip-side Protagonist", so I'm not sure how anyone missed that except it being translated differently on their pages.

Pretty sure she was feeling bitter about the whole ordeal, and not because she has feelings for Prez. She essentially made a fool of herself just to prove Kaguya wrong, and Shirogane was too in love to be swayed by her generous proposition, and Kaguya then acts like it's a major relief, and all happy.
Couple that with the fact she feels like she's missing out on her young love life possibilities, and feels like it would be nice to be loved just as much.
Miko's headed for some serious confusion and heartbreak even if she 'wins'. So far, Miko's been consciously supportive of IshiTsuba, more or less. She's lashed out before, but that was either when she was drunk or when she had an excuse that she probably believed at the time. Miko is a huge moralfag, and that's her primary complex. Not being mean to Ishigami, not being horny, and not being a loser.
Why would Miko steal Ishigami away from Tsubame after she realized her feelings? She's not going to be able to lie to herself about her actions anymore, which means she'll be consciously processing them as wrong and immoral. Boys and girls aren't even supposed to be within 50cm of one another, and she's got Ishigami hand feeding her. Hell, Tsubame has been nothing but kind to Miko. Chances are that Miko's just going to get depressed and sink deeper into neuroticism.
Tsubame said no. Miko is well within her rights to take all measures to get Ishigami and she wouldn’t be stealing anything.

He had a much better understanding of her than Ishigami does for Tsubame. As the Iceguya arc showed, he figured out the core of Kaguya's character pretty early on- that she avoided others to avoid hurting them, and that she tried hard to be kind despite her originally bad personality. He didn't know the details, but he always had a good grasp on what mattered most, and that's what he fell in love with- the real Kaguya, not some delusion.
Ishigami and Tsubame are completely different. He doesn't know anything about her beyond her being nice to him, and we know by now that there's a lot more to her than that. He pretends to be just like her other friends because he assumes that's what she'd like, and is completely blindsided by every little thing he learns about her (her love of horror, Tsubamenese...). The Tsubame Ishigami loves is a complete delusion, and it remains to be seen how that'll come together with the girls' character arcs.

I honestly don't think Miko needs Ishigami much at all. I just think it's the sort of situation that would put her through the wringer and squeeze out some juicy character development along the way. Her main issue has generally been forcing her ideals without taking other people into account and being considerate of them. Skills that her present infatuation with Ishigami could net her. Not to mention, it's really mostly that it'd be funny to see her act like a total autist the whole time.
With Tsubame, I think she needs to get over her trauma, or else it won't be a very satisfying story, but that's pretty much all from my perspective. I'm just not sure how broad and flexible an IshiTsuba dating dynamic could get. And post-healing Tsubame doesn't really need to date Ishigami either, and doesn't need to date him or have a perfectly functional and smooth relationship with a pure prince in order to heal. That also sounds a little one-sided and boring.
On Ishigami's side, I think it's harder to figure out what he needs. Giving up or closing his eyes would be the wrong thing, but trying hard isn't everything. Ishigami has mostly blindly stumbled into what success he's had so far, and from his perspective it's mostly been Tsubame approaching him. He's usually been too much of a pussy to approach her himself, no matter how big he talks or how cool he tries to look. Ishigami doesn't actually even know why Tsubame pulled him aside for some fuck. Are Ishigami's eyes really open? He doesn't seem to have a very clear perspective at all, and I don't want his success to be rooted in dumb luck and misunderstandings.

I'm pointing out the warning signs. Tsubame's interest in him was sparked by something he never actually had the courage to do, and which he probably still wouldn't have the courage to do.

Or to put things simply, Ishigami has been coasting on the success of a version of himself that doesn't exist, merely responding to the consequences of an event he is entirely ignorant of. That will have to be addressed eventually because that's not who he is. Not yet at least. Maybe he'll eventually grow into that kind of total Chad, and there'll be no difference between the real him and the false impression Tsubame has. Maybe things will get cleared up and Tsubame will realize that the real Ishigami isn't that brave, but still decide to go out with him anyway.
However, there's a certain ironic twist to this scenario. The Ishigami that Tsubame sees is a far more idealized version of him than the one who Miko sees. Miko's not the one seeing a confident Prince. She's seeing gross condescending pathetic Ishigami, but who has his moments that she wants to experience more of for herself. The delusional Miko is finally seeing both the good and bad in Ishigami, even if she has no handle on her emotions, with most misunderstandings cleared up pretty quickly. Miko knows that her "heart" was just some trash Ishigami picked up off the floor.
Tsubame is seeing a way cooler version of Ishigami than the actual. The misunderstandings that support this false image remain firmly in place. Unlike Miko, she's not aware of the true nature of the heart given to her.
The contrast between those two "confessions" is actually quite intriguing. Miko didn't get the big dramatic moment, but there was some mutual understanding.

As enjoyable as Prez and Kaguya were, it was like we were entering a race where the juicy part was over, leaving mostly a clear road ahead. Ishigami and Miko is enjoyable because we get to see the exact moment they're getting interested in each other, which we only had glimpses of with Kaguya.
They went off to eat while Kashiwagi guarded them, a good amount of time passed already, so I kinda doubt it. Unless they've been snuggling all this time.
After the dust has settled I'm glad the intelligent ones amongst us are in unanimous agreement that Tsubame is a huge whore and a complete waste of ink.

IshiMiko is a lot more exciting, a lot more tense, with higher stakes should you choose to get emotionally invested. Prez and Kaguya were already guaranteed from day 1. It was just a countdown. Ishigami and Miko are a shitstorm in motion and Miko the clear underdog in this setup, and it's still too early to get a feel for where the wind is blowing.
How will Miko's attitude shift as she gains self-awareness? How will she express her affection, and how will her perception of Ishigami change? How will everything triggered by Ishigami's accidental confession to Tsubame pan out? How will the false impression of confidence from that be resolved? Will Ishigami coast on good luck or change his approach?
These are just a few of the deciding factors.
