Spoilered for OT but Goku didn't use kaioken again until Super because the stress of using both SS and KK in tandem would likely lead to immediate death. He uses it again in Super because SSB has better Ki Control, but using it to begin with was a gamble that he wasn't certain would work in the first place. He DOES use it in the non-canon otherworldy tournament with SS, but that's because of the properties of otherworldly bodies. Also, because it wasn't canon.

The real-world explanation is because it was obsolete due to SS, but they do have an in-universe explanation for why he didn't do it.

For example, I came into highscool benching 165 lbs and as a senior benched 345 lbs, 2 years later I can only bench a little more (370). Obviously you can keep pushing your body but you do get diminishing returns eventually
