The servants can stay after the war, But they are no longer backed up by the grail, so the master would die after a few minutes/hours depending on how much od they have. Rin was able to do it because her Od is the amount of about 20 regular magus. so she's able to just barely scratch by keeping saber up with shirous help after.
And for medusa in Hf, Sakura Who is her master, is still connected to the grail. So she can fuel medusa after the way easily. for any normal person, its a death wish to try.

of course, if we take the scenerio where medea makes an infinite grail? i think that was a thing right during ubw. Then possibly it could work, they could all live after, bathing in their infinite grail B)

Also, Shirou only has to meet Zouken for the HF Route to happen, I can just ask him to accompany Sakura to our house one night and let Zouken know about it in order to be 100% sure.

If I'm in Fate, I'll just be best buds with Shirou and help him out. Take out Kirei very early (two months before the war), not fight Shirou and actually ally with him rather than try to deceive him, making the rest of the route a cakewalk. Zouken will die on his own once the war is over, which means that the worms will soon follow suit.
If I'm in UBW, I take our Kirei early, let everything else play out in a similar manner. Going back to the start, once Zouken gets the Grail fragments into Sakura, deworming her would be death. The only chance I have of saving Sakura is going to be contacting Shiki after Tsukihime. His Mystic Eyes are the only thing that could kill Zouken without killing Sakura. In the meantime, I'd build a strong relationship with Sakura so she'll have bonds with others, thus stopping her from becoming the Black Grail.
Kiritsugu is a loss. There's no way to cure the curse of the Grail, but I could link up Shirou, pool the Matou library with Kiritsugu's training and get both of our circuits up to snuff by the Grail War. If I get Shirou to learn about Avalon on his own and he learns how to use his circuits, he would be a godly force in the Grail War. Bonus, mage Shirou means Sakura won't hide her magic to him, so she can use Rider in full.

Sakura has significantly more and better circuits than Shinji's normal set as given by the OP. It follows that Zouken would give her the worm treatment as canon, and leave Shinji to develop his magical skills in some less useful way.

It also doesn't say which night this happens, just that it's some time before Rin summons Archer. Do you think you can search an entire mountainside every night for months prior to the War?
Maybe if you focused your training on scrying magic, you could use that to find her. It'd be tough, though, I think.

Between the three of us, possibly with Fake Assassin and a Rider too, taking on Gilgamesh should be possible and rescuing Illya is just a matter of knowing what to say to convince her she wasn't abandoned.
