It wasn't. The central conflict of the arc, the one almost every character cared about was who's gonna pass the exam. Shouto vs Inasa was just really prominent but it was only a side conflict. That's actually one of the issues of the arc: a lot of the shit happening had nothing to do with the only interesting conflict in the arc and vice versa. That why it has this noticeable filler feel about it.

That's shadowing. The real thing that occurred to me when reading is that this gesture makes no sense. That's a gesture for someone to take your hand in theirs and pull you up, which is a show of kindness, but makes no sense in this situation, for someone who knows Tenko's quirk. AFO isn't immune to Decay or anything, and also didn't have regeneration until after his first fight with All Might. He would have straight up died, his hundred years over, if Tenko used his whole hand, and there was a big chance of doing it even unintentionally.
Presumably it requires he touches his targets. If the hole here is what he uses then it makes sense that touches the two men in his brother's flashback on the heads, with his palm touching them.

If he knows his Quirk, then he knows how to avoid its activation. Since he's presumably fast enough to clash with All Might, he could probably just retract his hand before Tomura made full contact. He's extending his hand in "kindness", confident that Tomura can't hurt him even if he tried.
