The "world" in "different world" from "isekai" refers to an entire universe or such. Not just a different planet. jisho even translates isekai as "different dimension".

Shit, in Escaflowne they went back and forth between the worlds a few times and that came out in 1996. It's not like "being stuck" has been some staple of isekai.

Sao is literally a video game while Log Horizon and Overlord are "real" worlds based on the games.
