The "world" in "different world" from "isekai" refers to an entire universe or such. Not just a different planet. jisho even translates isekai as "different dimension".

The web novels ended in 2007 and they got republished as LNs after he won the Dengeki Bunko amateur writing competition with Accel World in 2008.

Shit, in Escaflowne they went back and forth between the worlds a few times and that came out in 1996. It's not like "being stuck" has been some staple of isekai.

Sao is literally a video game while Log Horizon and Overlord are "real" worlds based on the games.

because isekai has become genre fiction. as in it's defined by tropes and themes instead of it's literal namesake. in the same way a Western doesn't necessarily take place in the American frontier it's named for.
of course they live along and overlap with literal isekai not unlike the split between hard and soft science fiction.
