While destroying a hand, he says he wants to destroy two people who suddenly struck him, and kills them as AFO and the Doctor watch.

AFO gives Tenko the two people's hands as a present.

He has the four hands plus one to replace the one he destroyed before.
He then shows what he looks like with all the hands on.

While it is true Ochaco, like the other non transported students, had Kurogiri as her opponent, it is also true Kurogiri called Shiggy by his full name several times. Usually in hearing of Deku, All Might, and several of the students. They probably told what they heard, and the news media probably blasted his name all over Japan. What's interesting is that Deku could call out that name in a public place, and Ochaco was the ONLY one who reacted.
