“Burned to death” just means the body was burned and the person died. The exact cause of expiration is likely impossible to determine in such a state, so they just default to saying that. The few that died of “suffocation” probably did not have extensive burning over their bodies, so it was reasonable to write that in the autopsy.

No way a gasoline fire at the lobby would've killed half of the building if its stairwells followed IBC or OSHA regulations. I would have to go to the hallway to use it, though, which might kill me because of all that toxic smoke fuming through the public parts of the building. What do you expect, KyoAni should have had their own private fire brigade next door? You can fight and make measures against small to medium ordinary fire, not against that raging gasoline inferno.

People in one of the other threads were talking about how quickly the smoke spread in this instance, but I've had small fires start in my house a few times and even with those, it's crazy how quickly the area you're in fills with smoke. The smoke in my case was always comparably thin compared to what you see in the videos of this, but considering the fire here was hundreds of times larger and more aggressive than in my cases (at the least) I can see how the smoke would be a really big issue.
