It's not supposed to be healthy from the get go since they're both fucked up. EoE at least ended their arcs and showed they are capable of being vulnerable to others now. I like to think the former would happen since it's happier and them failing to connect again after everything would kind of ruin the whole message of finding hope in reality. But we'll never know.

These are things explored and fleshed out throughout the series and had an ending. Kaworu, meanwhile, is literally a narrative tool to move Shinji's character forward exclusively. I'm not trying to say that he isn't a character or is bad, just that he's definitely a separate beast from those two.

The reason why Kaworu is a white-haired pretty boy isn't just because "such is the other-wordly nature of angel-like beings." It's a conscious creative choice Anno made because he wanted him to be appealing to both Shinji AND the audience. He said as much in an interview about wanting Kaworu to be "loved by everybody."
Same goes for Rei, it's a common consensus that her design was made cute and unique looking as a reflexion on what makes a character appealing to otaku. Why should Kaworu be any different?
