It's ambiguous but the point of it was that both of them still wanted to be with the other one so it's a "good end" either way. They both give up the game on that pretty early with the snake woman story and his response to it.
There's no indication that she ever kills anyone though so the worst case scenario is they fuck until he dies, or he's in some kind of time distortion like in that Yandere Moth doujin.
I kind of wish we got an adaptation of this that wasn't hentai since it's still interesting without it, since the only part that's ever going to be animated is the first part otherwise.

After reading things like Mii Inside the Box that guy at least seems like he has some good times to still feel like he's got an alright result.
In his situation it was either be killed instantly or get the chance to fuck her for a while before it happens, so it still feels like a win in that universe. It also seems like he'd have the choice to try and escape or just kill himself which is nice.

I'm also reminded that Kyuusen no Shima exists so maybe Jyoka could go the same route as Higashida and go for a serialized horror story. But with better plot since Hades Island was kind of bland.
