There's a lot of ways that you can explain it, like one of Dio's lesser minions who couldn't see Stands but comprehended what was going on monitoring the fight or Pucci collecting information on Jotaro and siccing a bunch of Stand users on him shortly after the end of Part 3. The Pucci explanation is the easiest one since you don't need to invent new people to explain how it happened, but witnesses and/or Dio's underlings can also work.

The Flash is a fucking joke. Objectively speaking, there's no reason anybody should be able to beat him besides other speedsters. Force = Mass x Acceleration, so even if he's fighting someone stronger than him, he's able to make it up with more than enough speed. At least Superman has a weakness to a fucking rock.
