Multiple reasons. The series was originally titled Black but Kubo thought that wasn't cool enough, so he randomly decided to change it to Bleach. It should also be noted that the word "bleach" is synonymous with purification, which makes sense in this context because Ichigo's job early on was to purify evil spirits in Rukia's absence.

The initial idea was that Ichigo might have sped his way through to a bankai but much like Renji didn't have a grip on its abilities; his inner hollow on the other hand knew what it was doing. Ichigo was only confident enough to face Byakuya because he had achieved bankai. I agree that emphasizing the hollow as his crutch would have been less power bullshittery than his Bankai: Made Easy training, but that was what the subsequent arrancar and Hueco Mundo arcs would explore. Ichigo's bankai was still wasted and had issues, but the manga has bigger problems.

I think Bleach should have ended after Ichigo's fight with Aizen.
