That's the problem. If they just wait it out then the story and everyone's anxieties pretty much don't matter, which also holds true if they go whole hog in making them pair up. Not really. Satoko will be fairly old by the time Mashuu's a proper adult, and that'll bring it's own concerns in Japan. Not to mention that everyone who knows their past will be ambivalent at best about how she knew him as a little kid and there were concerns about her getting close to him back then. Even when he graduates high school, there's still plenty of room for anxiety.

Mashuu's problem is he grew up pretty deprived of socialization, at least when he was a little kid, and so he super idolizes Satoko for extending a hand to him and keeping him company when things were rough. Now he's older and expanded his circle of friends, but he's still operating like he has no real social connections or friends he should think about and just focuses all his attention on Satoko. He never really learned how to prioritize his social relationships properly and so he ends up seeming aloof and thoughtless. Which he is, but again only because that's what he's learned is normal because of a lack of earlier healthy friendships with his peers.

I think it's always been pretty obvious that their relationship is definitely going to be entirely non sexual at least until he's an adult. She has major self esteem issues and banging a teenager would definitely not make it better. Were people expecting this to go off the rails or something?

I think the fact the author made that strong statement makes people jump the gun and think that a romantic end won't happen. Personally I don't think she would spoil the ending like that.
