She understands Ishigami cares about her as well, but she's also too prideful to admit what a fuckup she is.
There's also the photos he showed her, which may have led her to realize he has been watching her for long enough to know how to make her smile, and for better or for worse she might have misunderstood that as affection of the romantic kind.
