He has a sea of LCL to live off of (see episode 1 saying it's highly nutritious) and as seen in the movies, Eva pilots age much slower. He has enough time to build a grave for Misato at the very least.
The experience of living alone in such an environment has made him a tad unstable, and when Asuka first appears he's not sure if he's still inside instrumentality. If he is inside instrumentality, Asuka won't react when he strangles her (as was the case when he was inside it) as instrumentality is your ideal environment and nobody rejects each other.
However Asuka makes an unexpected reaction, and caresses Shinji face. This demonstrates that this is indeed reality and Shinji isn't hallucinating. Upon this realization he breaks down and cries.
As for Asuka's final line, it was basically decided by her VA who decided to comment on Shinji fapping earlier to her + the fact he's crying like a baby. Hence how disgusting.
