The manga jumps the shark so many times and has so many insane plot "twists" and revelations that they had to cut it down just to make it only as ridiculous as it was.
The music box is annoying though because it gives them another opportunity to use the song they already over-used by a factor of 100.

First and foremost because the director was a hack who didn't get that Elfen Lied was supposed to be silly at times and removed anything that got in the way of his pretentious vision of a DEEP anime about social injustice, nevermind that incontinent people such as her definitely do face social mistreatment on a level easily comparable to the other characters.
Also, most of Nozomi's major scenes are in the second half of the manga that was still ongoing while the anime was being made, not to mention finding a seiyuu who can both sound like a timid girl and sing convincing German opera is a really tall order. That said, Nozomi is best girl, and the anime is shit for not including her.

His original idea for the whole thing came about from thinking how humans might have loathed and despised the "missing link" so much they killed them all, and extrapolating that to something coming along that's like humans but a bit better, and wanting to do the same to us. He didn't start from a love comedy.

You see this a lot with early 2000's and on back anime. Everything had to be super serious and more edger than the manga or source back then. It really was a different time.

It would have been a lot more obvious to others that she needed them if she did that, not to mention she feels terrible enough just about wetting herself, so she would try to use the bathroom whenever possible. We see her going into the bathroom to take it off at one point, too.
