alright I'll take a crack at an isekai plot:

standard run of the mill "everyone's in a video game" but when the isekai happens, the MC just so happened to have been testing out his hacked client.
Every attack is a crit, he has forcefield auras on so that if anyone takes a swing at him from any angle he parries instantly without even noticing them, he never misses, ect. ect. ect.
The "Overpowered MC" format already has proven itself to be pretty fun to be apart of so a video game equivalent of one punch / mob psycho

Isekai that has all the clichés, including, but limited to: demon king, shy megane with big tits, loli with big tits, tomboy tsundere with big tits, magic high school setting, battle harem, et cetera. Any cliché you can think of, it has. The only difference, however, is that the hero is actually gay and ends up falling in love with the demon king. Together, they obliterate all the generic waifubait characters and conquer the world.

Everybody playing a generic mmo when it shuts down gets stuck in it. They actually play it like an mmo instead of all of the other isekai where suddenly mmos are these magical things and not just stacking in a specific corner for each encounter and spamming aoes.

MC uses Isekai knowledge to practice developing his new powers. However in doing so in isolation, he ends up developing rule shattering abilities. Not overtly powerful abilities, but ones that break the rules. So while the MC isn't dangerous in terms of fight, but he represents a huge risk in other ways that people would go to war over to get their hands on.

The MC sees the world as some magical place to explore and learn, while everyone else sees it much more worldly and serious. The story contrasts these two viewpoints until they come together fully and the MC is a bitter man whose less than he started despite gaining so much, almost forgetting the original reason he started, to have fun and meet friends.

It’s an Isekai where the new world exists in the dreams of the protagonist, and when he falls unconscious in this dream world he awakes in the “real” world. He begins to lose his sanity as he can no longer tell which world is real.
He meets someone who is like him and further gone. They kill themselves in the “real world” to stay in this fantasy land, and when the protag goes back to dreamland, he finds the suicider’s corpse. Plot is some bad guy wants to merge these two worlds that used to be one and rule them. Worlds begin to affect one another. Eventually villain succeeds, but protag kills the him and saves what’s left of the merged world but most of the people from both worlds he grew to care for are dead and he is a broken man.

Make an Isekai world. That is, for every possible circumstance for reincarnation, you are brought to one specific world. This world is solely comprised of isekai’d overpowered protagonist bait, each with unique abilities granted. The world is shit since the people who run in are basically incompetent loser neckbeards (no guilds because no one needs them/ knows how to run them). MC doesn’t have a (good) power, but uses his latent leadership skills to try and unite the reincarnated population as a functioning society.

Two worlds used to be one world but alchemy/Atlantis/Xanadu/Mu separates the worlds in two thousands of years ago and they develop very differently.

Harem series where the MC is gay but not of the girls realise it sounds like it'd actually make a good SoL comedy.

His remaining friends try to get him a job somewhere in their village, but he immediately gives up whenever he shows the slightest sign of failure. Eventually he gets reprimanded by the one person left who's been helping him out, and they get into a fight. The NEET loses, and he runs off to his house on the edge of town. 6 months later, the NEET regrets pushing away everyone, and tries to convince himself that he can change. However, he makes no attempt to reconcile with anyone out of shame. Instead, he sets out on a solo journey and is soon attacked and killed by wildlife. No one notices until a couple of weeks later.

Within 100 chapters, he power levels his way to the demon king, kills him after a drawn-out and well-written fight, and goes home.

Along the way, a group of Guardians(human agents of the gods) catch onto him, but after mentioning Primal Unity, the highest ranking among them dismisses the other Guardians and starts trying to question the MC about what he knows. MC admits to knowing nothing, and the Guardian struggles to think of what to do, before deciding to "help" MC on his journey. He creates a beam of light into the sky, which summons forth a girl in pure white armor who appears to be slightly older than MC.

"This girl can take you to your parents, or she can take you to Primal Unity. There is no point in trying to stop you from reaching your goal, but we can try to prevent you from going down a dark path in the process."

So the bad guys can prepare a huge space armada loaded with relativistic weapons to fuck up a hard sci-fi world, only for their ships to go Dead Space on them due to otherworldly abominations isekai'd from the horror world.

When they finally meet, they are right in the middle of an invasion that they caused, being the rulers of both nations at war.

MC gets sent to another world, but instead of your bog-standard JRPG fantasy world, it's a far-future Science Fantasy Mecha Anime world. MC excels as a pilot because the controls for the mecha are eerily similar to his favorite "Totally Not Virtual-On" knockoff mecha action game, but rather than stick around and fuck hot space human and space elf babes, he'd actively trying to find a way home.

He eventually returns to Earth, but it turns out hundreds of thousands of years have passed, and not only has everyone he's ever known and loved died, the Earth is a lifeless death-world. It also turns out the world he returned from is in the same galaxy, so his space elf/human waifus finds him and brings him back home.

So not only is he trapped on another world more or less, he's also trapped IN THE FAR FUTURE, well after Humanity has taken to the stars and left the Earth behind.

So then the MC's next goal is to figure out how to time travel to the past because despite their super-advanced tech, no one in the entire goddamn galaxy has figured out time travel.

MC eventually succeeds, but find his normal, boring life unfulfilling, and is riddled with guilt at leaving behind his waifus. So he signs up for an ambitious project that will put him into cold sleep as part of the FIRST EVER expeditionary force to a far off, distant planet capable of supporting life.

Then we cut to the waifus back in the future still grieving over the loss of MC-kun, only for them to receive a distress call from an unfamiliar ship and it turns out it's the SAME FUCKING SHIP that the MC ended up being loaded onto ages ago. Girls save the ship and are unsurprisingly reunited with the MC who hasn't aged a day because fancy cold sleep technology.

At this point richboy believes that slumboy has caused too much damage while slumboy has given up on getting richboy to see his point of view.
It all leads to a final clash between the slumboys faction vs the kingdom.

In a fantasy world, a great hero is dying, and a cleric performs a resurrection spell. For some reason, the soul of a Japanese kid is brought to the hero's body instead of the hero's soul. The other characters realize very quickly that he's not the same guy, but try to keep a charade because the hero was a big deal. At first, he's pretty happy with his cheat skills and the harem of women around him (he was engaged to a woman already, and also the cleric who resurrected him was in love with him), but when some of his enemies realize that, despite him having the same powers than before, he's way more clumsy and stupid, he starts having trouble fighting. Also, he does some asshole stuff that makes people start turning away from him, and even the girls slowly stop liking him, even the ones that don't know that he's a new guy. He starts to crack under the pressure of being the hero, some drama arises about if he should care about living the rest of his life following the seemingly perfect previous guy's steps (drama that I can't for the life of me write down, I bet you can imagine something better than what I could come up with). Possible resolution, the soul of the dead hero starts to wake up again when our MC does heroic shit with his body, in the few moments that both the hero and MC are able to coexist, hero reveals to him how insecure he was deep down, eventually more drama arises when MC realizes that, if he starts becoming a true hero, the real hero's soul will come back in full force and he would have to leave the fantasy world, but he eventually does it after learning about what true heroism means. Possible sequel, back in the real world, MC quickly goes back to being a whiny bitch and discounts the entire experience as a dream, until the hero's spirit starts to communicate with him in the real world.

In despair, he manages to escape, thanks to having learnt from his owner how the restriction spells work. Eventually frees other slaves, including cute girls that are isekai'd too, and used as sex slaves, and starts a slave uprising, and in the end he turns his previous owner into his slave, because he didn't learn anything and still wanted a slave.

Turned out boy's family has a long history of her 1st-born dying young and getting isekaied to save another AU. They know this because apparently one of their ancestors became powerful enough in the AU to send a message to them through time/space.

Girl is devastated but still determined to find a way to re-unite with her one true love. She spends all her life training in mortal kombat, spec ops and deep cover espionage just in case she ever find a way to isekai as well.

One day she finds a weird cellfone app download that supposedly allowed people to speak to another fantasy world called Deus Ex: Love-Love Revolution. Not minding the strange IMPLICATIONS,she installs the app and tries contacting someone from the other world. Strangely enough, an ork prince contacts her and immediately confesses to her after chatting a whole night. He even claims he will order his royal mages to conduct a large scale ritual to isekai her over to his world.

"But user-ork-kun, I am already an old xmas cake of 30 years old!"

Ork prince doesn't give up and tell her to prepare herself the tomorrow night. She gets isekaied but apparently something went wrong and she was teleported a hundred years into the AU's future, and the ork kingdom is already destroyed, and she somehow transformed into an elf in the AU world in an elven village. Strangely her Deus Ex app still worked and she tried contacting someone again. Yet again some guy confesses to her and even claims he was the reincarnation of an ork prince. Finding this hard to believe, she hangs up. After living a hundred years as an elf, she accidentally discovered the hidden history of the dead ork kingdom. It seems that the ork kingdom developed a culture similar to nippon and even invented mayo, and it was all credited to...

All humans are isekai'd into other realms of Ygdrassil after the earth was destroyedd by an unknown evil force.
After only 100 years some of the other realms like Wannaheim or Alfheim now have big human cities like New York, Dubai or Tokyo.
Humans also have developed a highspeed train that travels from one world to another to connect the new metropolises with each other.

Show starts in a Sci-Fi Gundam sort of setting. You're given a cursory introduction to a trio of characters who're rebels of some description against an evil empire. They're clearly not winning the rebellion. Their current trump card is a repurposed Imperial war-drone. We're talking high tech autonomous fucking murder bots. They're reworked its brain to act as a bodyguard. Shit goes south at the end of the episode and the rebels get fucking swamped. Soon only the gruff 'too old for this shit' veteran and the Warbot are left.

Vet get gut shot, falls back into the bot. His dying brain hanging on his desire to save his allies. Suddenly a blinding light zaps both away. Cut to mages tower under attack by bandits. Students in the school attempted a summoning spell hoping to get something to save them. They get the drone, however in a twist of fate the summoning is jumbled. The Vet's soul being layered into the network of logic and numbers that made up the machines mind.

It is summoned with a single train of thought. "Protect them".

Queue the absolute worst things Bandits could find behind a door they've kicked in.

The skeleton crew considers this as an insult and are constantly attempting to be as spooky as possible in everything they do. They hate that nobody can notice they are spooky.

The struggle to maintain the facade and satisfaction levels is just as important as the bloody battles taking outside the town but now a major threat has appeared in the form of a precocious little boy who's noticed something's up - the world is not what it seems and he's going to make that clear to his beloved hometown.

Three dimensional displacements which all eventually culminate with the god who started the whole shit for laughs getting fucked up by The Master (Richard Gray) who had isekai'd his own psyche into the god's realm.
