To answer your actual question, she made her wish the way she did because it was important to her to make her wish specifically for him. She wished for this specific miracle because self-sacrifice is noble in her eyes. Wishing for a super-power is not. Her pretensions to nobility is seen throughout her story but especially in the climax. Unlike what some RETARDS will tell you, she isn't sad because she got "dumped". That's part of it, but mostly she's sad that she failed to live up to her ideas of virtue. One of the turning points for her is how she briefly considers what would've happened had she not saved Hitomi. That selfishness of hers disgusts Sayaka and more than anything that failure to live up to her standards is what makes her despair. She's a flawed and tragic but very fascinating character - my favourite in Madoka and one of my top favourite pop-culture characters. If characters were to go through the plot of their stories with a cool detachment like some fucking Isekai protagonist then all we'd have is Isekai writing. Personally I like my anime to be interesting so I want my cute bakas to stay.

Although maybe there was some hope in the end depending on how you interpret it. Maybe Madoka is a story about a girl who becomes Jesus and gives peace to a bunch of Fairy tales that all had bad endings, and the fairy tales are represented by different girls. Kyouko was Little Match Stick Girl.

I think she did know but she wouldn't be honest with herself about it because she thought contradicted the ideal she was striving for, a strong and selfless hero as she believed Mami was. Underneath Sayaka was an insecure teen girl who was very hard on herself, her well-meaning but misguided struggle to do good resulted in her compounding on her woes until she died in nihilistic despair. Yeah she was a baka but she's sympathetic, she had an impact, Madoka gives her a heartwarming sendoff, and the way she is at one with her witch seems symbolic of her being more at one with herself.
