I can't think of very many minor villains off the top of my head that weren't just killed or incapacitated. Angelo was going to be a rock but I think the second time Josuke punched the rock Angelo was killed. Maybe not though.

Kars eventually made it to Mikitaka's planet and destroyed it, causing him to come to earth, meet josuke and save him from Superfly.
Kars inadvertently saved Josuke's life.

It’s common knowledge araki has a creator boner for part 4, it is not surprising in the least he gives its characters special treatment (thus spoke, DMQ, literally part 8).

He's the bad guy who keep the worse bad guys from the door. He went from a serial killer to an antihero. Stressful or not, that's cake compared to a typical villain death.
