Good source of tension if handled properly, but only truly effective within the context of a grounded romance. The relationship losers in more action-oriented series mostly just end up being there butt-end of jokes or relegated to irrelevancy. The final confirmation of the loss is also something that should only come toward the end of a series unless the series or author is some kind of miracle (i.e. Jitsu wa Watashi wa). Your pic related did a pretty good job of being a designated loser. In retrospect, another function of such a character is providing insight for the reader and also for other characters.

Should be built up strong so the person meant to win looks good by besting a truly worthy opponent who felt like they actually stood a chance of victory. Too much romance makes the designate winner a mary sue with tacked on issues for cheap sympathy points, while the designated losers are the underdogs who never stood a real chance and may garner actual raw love and sympathy from the audience.

I hate it, especially when they refuce to actually let the losing girls move on, they'll always be there alone with a sad smile as the two love birds are ignoring everything around them to get lost in each other's presence and eyes. It's even worse when the girl won't take a hint and becomes an antagonist for it.

If you go by the usual standard that most anime/manga romances only really get resolved, or fully manifested at the end, then any love triangle is fundamentally a series-long filler arc. Romance for a lot of series, especially stuff like action shounen manga, literally just exists to pad out pages and character interactions because more people are interested into rooting for a girl(or guy, as it were for shoujo stuff) to win than actually seeing people in a relationship. People like playing the game, people don't really care about the CONGRATULATIONS screen at the end of the game.
Beyond that, for a lot of romance manga you're basically looking at a girl getting anguished over your generic self-insert with the qualities of

I mean, I get it, I was a horny teenager once too and I've been rejected and dumped a number of times but damn, girl, save that energy for someone worth it.

Really, it's just annoying to see the cast get filled out with characters whose most important contribution to the cast dynamic is "in love with MC because she has to be" and then just rinse and repeat until you get a light novel/harem series.

Can certainly work, but there needs to be an idea for what you want to do with them. Otherwise if they end up just loosing with nothing gained out of the whole ordeal, it just feels mean and anticlimactic. It can definitely add a lot to the conflict, though.
Imagine loosing to your lovers right hand. She really deserved better.
Oddly, I've noticed this is only if the MC remembers that childhood friend. If she's a childhood friend the MC once was really close with but then forgot about for some reason, or who is unrecognizable, then her chances skyrocket and she's nearly guaranteed victory.

That's just how it is, the loser is needed to tell the story.

If there is to be some conflict, there has to be someone who wants the same thing.
It has to be shown that the target character is "desired", because someone wants her.
If only two characters would be interested in each other, everything would be taken for granted. There have to be other options to have a worthwile story.
And love stories are about finding something that changes your life and gives it new meaning. Something has to change, so there has to be an old status quo. In this sense, the loser is kind of the same as the "designated friend" characters.

It can turn out nicely if the loser gets to find someone for themselves in a believable way.

It depends. In Sayaka's case, she wasn't a good match for Touko, and only really liked her for the looks. She knew Touko wasn't happy but didn't care to do anything until Yuu showed up then promptly started acting all haughty. She deserves to be a loser.

In Citrus for example, I think Sara is a better match for Mei than Yuzu. The two complement each other perfectly. It's a shame but there's really no point in shipping when the end result is obvious from the start.

Yes. I think that was implied in the chapter that Touko decided on Yuu. She simply waited too long, and before she made a move Touko got someone who was right for her, at the right time.
