Identity death is the complete replacement of the original personality by another one. Mahiro's behavior might have changed somewhat but she still has her former memories and continuity from her previous personality so she's still the same person.

I do think that more genderbending series should have the protagonist reflecting on the effects of the transformation on their personal identity and on whether they're still the same person as before though. You'd expect someone in that situation to do some soul-searching, especially if they realized that they were thinking and acting differently compared to before.

Mai has a good character arc and the manga did a nice job of showing how she adjusted to her new environment and to life as a girl. The fact that Mai spent time with other girls instead of her being roped into a 'will they or won't they' relationship with a dude was nice as well. It also treated the genderbending fairly seriously as Mai questioned the implications of her transformation on her identity and didn't have dumb gags you see often in genderbending such as the protagonist not knowing how to pee or thinking that they are dying from a mysterious disease because their vagina is bleeding. The fact it had great art and good fight scenes were also advantages.

The only characters who called Mai that in Variable Heart were Kagura, who's lecherous as hell, and Ragna, who likes to come up with annoying nicknames for others (such as calling Bullet ass-woman). It's two characters who have shown that behavior in previous works, not everyone.

I'm surprised I've never seen one where the joke is that the genderbent MC thinks they're just having a normal period for the first time but they've actually got something horribly wrong with them and they're like 'wow, girls all go through this every month, huh?'
Although on second thought that's probably further into gynecological body horror than anybody really wants to go
