How does a character designer of an anime that's an adaptation of a manga or LN work? Aren't those characters already well established already, what exactly is he "designing"?

It's impossible to get a 100% replica of the original author's vision for an anime adaptation. He's there to try to get it to as close that as possible.

He's drawing how the character would look like in the adaptation. He's tweaking the design to fit certain stylistic choices and creating a coherent design that could be worked into the anime. Some designs are as simple as redrawing the character, some designs are about completely redefining the style of it.

The original designs have to be redone with the purpose of animation in mind, since the original manga/ln designs are done with the purpose of stills in mind.

not him but there was probably some production that did that, although mangakas rarely touch animation or the animation process. Rarely will the mangaka understand enough about the animation process to draw out characters to be animated.
The only mangaka who did character designs for the anime I can think of is hayao miyazaki with nassicca.
