Him spilling the beans to Shaft and possibly asking that they include the foreshadowing doesn't mean that he designed the ops/eds himself or had any say at all with regards to particular imagery. He probably only gave the okay to use his picture and nothing more. FYI this was the same picture used in his living funeral. Most likely Shinbou or someone else (a producer maybe?) thought it would be a laugh to include it. I doubt it was his idea, but I guess we'll never know.
