I've long thought Farnese might be the one to use it. Among Guts' new party she's been among the most focused and developed since her introduction, and as a result has changed the most. From a story perspective, the reason for this is likely to make her function as Casca's caretaker and demonstrating what a budding magic user goes through, making her useful and providing a clear character arc. But with Casca recovered now and Farnese getting a good grasp on the basics of spellcraft, those angles are pretty much settled. Looking at Farnese now compared to where she started she's almost like a different character, with her only remaining insecurity being her infatuation with Guts. So now would be a good time to throw a curveball her way.

There are some neat options for how it could go. Guts is branded so she can't offer him, and that leaves the obvious choice in Serpico and the slightly less obvious Roderick, who's been shown to be genuinely charming and capable of getting her attention (after the dreamworld and with Casca awake he has a good chance to win her over). Between the two of them I'd think her sacrificing Serpico would be more likely, given that he hasn't changed much from his introduction and has sorta stagnated as the babysitter's babysitter. There are flashes of being a rival to Guts but Serpico's passivity stops it from going anywhere when Farnese isn't involved. Having Serpico be on deaths door anyway and imploring Farnese to sacrifice him to save herself would be a good way to cap off his life's journey of looking after her, assuming he doesn't finally break that mold soon.

Farnese doesn't have any reason to use the Behelit, she now has found a purpose in life that fulfills her -- being Casca's bodyguard. What do the God Hand have to offer her at this point in the narrative? She is content.
Casca, on the other hand, is currently experiencing the 2nd worst moment of her life, which is probably tied for the 1st worst moment in her life given that a large part of the day's events are reliving the Eclipse. And in addition to that forcibly coming to terms with the 2 years' worth of humiliating experiences she's had as a potato. And in addition the knowledge that she doesn't have the superpowered strength or skill in magic to actually be useful in the upcoming revenge quest against the monster army that Guts intends to walk into with Shierke. Casca is in a shit position and is in urgent need of power, which the God Hand are only too willing to provide for the low low price of a sacrificed loved one. Loved ones who so conveniently are living testaments to her shame.
