But then again, I think he was just getting tired and wanted his clusterfuck series to end while it was going good. he didn't actually lose it though.
he just time traveled back to the present. as it happens, in this new timeline he was in a coma until he 'arrived' as his destination time but the only price he really paid was having to go through physical rehab, which is a pretty mild karmic punishment for a story about abusing time travel to achieve a 'golden end'.
like even if he didn't have the coma, his personal experience would have been much the same; he zaps back into the present but now the timeline is all weird and his personal experience of those fifteen years contradicts the history everyone else experienced
I feel more sorry for his mother who had to watch over him and make sure he was taken care of for fifteen years with no signs of him ever waking up.
at least with the coma, him not remembering any of the important events in his friends and family's lives in those years isn't something they'd ever notice or question, and he'd never feel like an imposter that just took over someone else's life

Its explained better in the manga. MC "Lost" those 15 years as karmic balance. Basically he traded the 15 years of living his life for the people who died to live the 15 years they wouldn't have otherwise. The mom wasn't part of that trade as she already lived those 15 years and got killed after that. Her tending to him for those years may have been her own balance for not dying later.

From the start of the story, Satoru's romantic interest has always been Airi.
Airi was not supposed to "exist" in Satoru's new corrected timeline yet the two of them meetup by sheer coincidence in the final scene, that's a well justified ending to tie up loose ends if anything.
