Naruto is a weird case because they're all ninja with Japanese names and the world itself doesn't seem to have any non-Japanese-y parts, but there are also pretty spectacularly non-Asian people running around, title character included

Generally speaking I just assume that the author doesn't put any energy into making physical traits consistent with real world populations VS just wanting to draw characters a certain way, like with how there's that one black dude in bleach despite being a character with a Japanese name in a distinctly Japanese version of the afterlife.

Then you have weird stuff like how in One Piece, you have Zoro who wears Japanese style clothing, has a Japanese flavored fighting style and Japanese swords but there's an actual Japan proxy country out there that he has no relation to and doesn't visit until like 90 volumes in

Basically: characters look a certain way because that's how the artist wants to draw them. There is rarely any more thought put into the issue than that. Even in Kengan Asura, a book that takes place in a relatively realistic setting with a cast that is 80% Asian, you have stuff like half the Asian characters having naturally blonde hair.

Anime characters race is determined by the context of which they are a part of. Show based in Germany? They're probably German. Everyone has Japanese names? They're Japanese. Trying to determine race based on appearance is stupid because no normal human looks like an anime character, stop trying to make this a thing

Then they are whatever you would would call the ethnic/racial group from asia. Personally, I use the phrase "oriental" to refer to japs, chinks and koreans, which is pretty much what the majority of people in Naruto are. The only people who aren't are the handfull of negro characters from the Cloud village. Apart from them, every other character is the same race.
