The manga WILL end in a final showdown between Natsuo and DIO. In stopped time, DIO will throw his lives not at natsuo himself, but at Rui who is a bystander in all of this. Natsuo only has enough time to put his body between the knives and Rui before time resumes. Natsuo is impaled several times. Natsuo's last words to Rui is him telling that what he said in the past was true, he would take a knife for her just like he would for hina, and he loved her so much more he'd take as many knives as possible to show his love. The final blow is DIO throwing Rui's knife to Natsuo's heart, killing him dead in a cruel twist of tragic irony. Rui screams out in sorrow.

In the end DIO is victorious and Natsuo is dead. Credits roll and Holy Diver plays.

This is proven even further by this . It shows that Natsuo and Hina have always been the MCs while Rui, who doesn't even appear as herself in the second or third scenario, is irrelevant in the grand scheme of things. She's just there to create and fuel drama. The older woman x younger guy concept is the main idea making Hina x Natsuo inevitable.
