Hell, maybe he was Quirkless. If Deku's presumably 16 (a year HAS to have passed) at this point and Shigaraki is 20, then Shigaraki could have been 4-5 when it happened. If we tie it into Dad AfO, maybe Izuku's prenatal stages were a testing ground for developing quirks and Inko didn't even know it.
Hell, maybe as a standalone theory Inko's had her memory fucked with ala a Quirk and AfO did awful shit to her that was then erased.

Unless some 3rd/4th party saved him, he either bled out or was captured shortly after. I mean the LoV blew up a prison transport, that would be reported pretty fast, so unless someone just saved him, he'd probably be in Tartarus. But yeah, we don't know.
