Your last 2 points seem to contradict each other. If that were the case it'd be too much like the Green Baby thematically, though Araki has reused stand powers before. You can't really predict anything based off a Stand's first appearance in JoJo usually. It makes it even more grueling that we have to wait 4 months just for a new volume release since the story doesn't progress for shit in 1 chapter.

I like the idea someone posted before that the stand only exists when not detected and the doctor only exists when he is detected. I actually had a dream last night that this or some other stand had basically an "author" power and was modifying parts of the story/the reality of the story which could explain things like Kyo disappearing from the photo or Tsurugi being in two places at once during the gate incident.

The stand we saw is definitely Tooru's stand though. Its "eyes" have the same sun motif on Tooru's clothes.

1. This Rokakaka fruit seems to be the pinnacle fruit. For some reason this branch determines everything for the Higashikata family, Josefumi, ties back into Johnny, etc. I believe the Akefu Satoru is actually unknown from most people, and has finally shown up because this fruit is the last fruit he needs to execute his plan.

2. We haven't seen Holy mentioned in a while, I feel like this is going to be a segment where she comes relevant again, and has something to do with this Akefu person.

3. The significance in his age is either a meme or indicates something bigger in the plot. It seems the older Higashikata family members have something to do with each other, and this character may also have something to do with them.

Theoretically speaking it could function similarly to Mendel's experiments, the parents being Josefumi and Kira, Gappy being their first generation off spring and depending on the dominant genes passed, would display more characteristics from that parent but will still carry both's DNA in him, with enough breeding all characteristics passed down from the parents would show up eventually.

For the ones saying that D4C lasted many years, that’s because D4C was 3 fights in a row: Diego & Wekapipo VS Valentine, Diego & Hot Pants VS Valentine, Johnny & Gyro VS Valentine. That’ because, up til that point, Valentine didn’t had the necessary amount of screentime (for a main villain at least) and never used his stand in combat.
Jobin, on the other hand, apart from a good chunk of screentime (particularly his flashback) also have 2 arc: beetlemania and ozone baby. So his final battle will be 10 chapters, more or less (the same amount of chapter of Johnny e Gyro VS Valentine)

Guys, what if Tooru's ability is to cause people to see illusions of this Head Doctor instead of the HD being a completely new character? I think this could work as a possible reasoning for everything that happened in this chapter.

Why was the HD there right after it happened? I think it may be something as simple as Tooru trying to frame Tsurugi for the murder of that girl(plan that failed). Now, how did he do it? Ok that's great but why is the HD there? Well I think a "weakness"(not really a weakness per say) of Tooru's stand is that he can only create the Illusion's in a short radius around him but the stand can't go too far away from either Tooru himself or another illusion. Now, why would he do that? Well I think he is trying to kill all of the Higashikata because he knows they have the new Rokakaka branch in their estate.

I like Jojolion. I don't care how long it lasts. We're only 11 chapters behind SBR and I don't expect it to conclude in that amount of chapters. JJL is still 861 pages behind SBR though. Assuming an average of 45 pages per chapter it'll take a little over 19 more chapters to reach SBR's page count, which I think is likely.
