Its right in the middle of chapter 165 - Nakoshi convinced Nanako to allow him to trepan her after he "rescues" them from a ganger Nanako stole (maybe i dont remember, details are fuzzy) money from. Where Manabu operated on Nakoshi almost surgically so, Nakoshi just guerilla drilled a hole in her head.

Shit, that reminds me of that manga where the girl shoots at her head with a revolver so she could touch her exposed brains.

I forgot to mention I'm pretty sure he survived, but never made a complete recovery.

Had something like this happen to a classmate in woodshop. They slipped while using a band saw and got the blade wedged pretty far into their hand. Instead of pulling back, they yanked their hand to the side, which snapped the blade and took a good chunk of their finger with, exposing some bone.

When I was 5 I had my pinky finger crushed in a school door that had a fucked up lock. Some kids had worked it open earlier but shut it on my finger, completely flattening it. Because it locked again after getting shut, it took a few minutes for a teacher to show up with a key to unlock it, open the door and remove my finger.
Unfortunately I was absolutely freaking out and trying to yank it out for the first few minutes, when the door was unlocked and it was finally removed, my pinky was thoroughly mangled and the bone was destroyed.
A doctor actually managed to reconstruct it though and I now only have a small scar, the fucking miracle worker.

Less gruesome and more an event that spooked the fuck outta me in high school in my own woodshop class, a rubber glove tore off an inoperable sand blaster that was being stored in the shop, being the absent mided fuck I was I was walking around the shop room with it on my hand.
For some dumb reason I managed to touch the sanding wheel with the glove.
It promptly slipped off my hand and became a solid and compressed piece of rubber jammed between the wheel and the mount, seizing the machine.
The teacher was just glad my hand didn't stick inside the glove and go with it, it's likely I would have destroyed a good portion of my hand if that happened.
That event was more of a learning experience than probably the rest of high school, I'm now a bit paranoid with safety protocols and common sense around heavy machinery.
