On his way home, MC looked at a nearby park and saw a girl in an old patched up school uniform holding a sign that read 'Bodyguard service, 50 yen/day'. It was very cheap, but she looked serious about that.
He thought: "Well, I don't need protection right now but at least I can hire her to keep me company like a girlfriend".
MC was about to pay her when suddenly a gang of thugs showed up there asking for their money. Without a second thought, the girl ran toward them but she tripped at the last moment, the thugs laughed at her.
Luckily, MC knew some martial arts so he beat them up so bad they fell unconscious. She thanked him in a very quiet voice while blushing a little. Her tummy started growling so MC invited her to eat ramen.
From that day on, she started following him around trying to protect him from danger, but he always saved her instead.
The girl is a kuudere and a total klutz (something that you don't expect from a serious character), she's also very poor: she lives in a makeshift cardboard house and her diet consist of bread crusts and water from a fountain. Pic related is a crude drawing of the girl that I made in paint.

A tribe of people live on the backside of a whale/mountain/giant beast wandering a flooded world and are in the process of emigrating their people group by group across any islands they come across because the giant they live is going to die.
The story follows a pair of kids who have the ability to speak with the giant and communicate its anxieties and desires to the tribe committee as its chosen priest and priestess. It is their mission to repay the giant for saving them years ago and allowing them to live on it till now. Unfortunately for them, the years of enduring the flood and providing for the humans have left it terribly weakened and its end is coming so they are all living on borrowed time to ensure the likelihood of humanity's survival after its death.
While the elders manage the logistics of the emigrations, the main characters do their best to bring relief to the people and the giant by generally making good memories for them before their final farewells to each other. They'll play songs, trade stories and explore what they can so it's mostly a post apocalyptic slice of life that slowly shifts into a story where the kids do what they can cope with their big friend dying in front of them and being helpless to help it.

What's the point? A desk can't interact with the world in any way. Either find a non shiba lifeform that can interact with this world in some way, or just don't make it an isekai and make the psychiatrist the MC. One episode, one patient. Alternate episode where the MC develops with episode where the patient developps.
