The general tankiness seems to be a sum of core defense muscles and skin provide + invisible haki coating that makes your body harder. Kaido tanked G4 Luffy withour haki cause he was drunk drunk and with haki he's probably tougher. We didnt get to see BM get damaged without haki cause the assasination plot failed but it might be that her haki is just so strong but her body is soft or that she's just as tanky without it as Kaido. Pangaea is the name of Mary Geoise castle but it's also the name of the former super continent on
Earth. Oda wants to keep the mystery on the choice of that name.

The theory goes as followed:
During the void century after the 20 allied kingdoms won the war and formed the world government they split the world apart in order to prevent the rise of any other power : they destroyed the One Continent (one piece) in a bunch of little islands with the help of the Oars race (demons as big as islands at the time). One of the remnant of those demons is the skull that forms the island of Onigashima near Wano. The demon race ended up almost extinct and greatly diminished in size with Oars and then Oars Jr (barely 30m tall). Kaido is also a descendant of that race, explaining why he's the 2nd biggest human in the world and why he has those horns.
