It was a mistake, but I think some people make it to be way too big of a deal. I hardly saw this mentioned before the final chapter so it seems like people just clung onto it for a reason to dislike RR or to call it unhealthy or whatever. Even after that incident, Rukia knew Renji was worried for her deep down inside. It also always confused me how people hate Renji for that moment and don't want him near Rukia, but are fine with Byakuya and his relationship with Rukia despite the latter being 110% set on the execution until Ichigo beat some sense into him. They won't forget that one act of Renji, but wave off Byakuya's act and indifference/coldness towards Rukia for over 40 years despite his behavior having an obvious affect on her self-esteem.
