In Brandon Lee's case they didn't have a licenced gunsmith handling the props. The accident is actually the reason Hollywood now necessitates a designated armorer to be on set every time a gun is handled. Baldwin also isn't just the guy holding the gun - he's the producer which means he has a lot of responsibility for how the production was handled, and IIRC he kept hiring non-union scabs for a lot of key positions, including the propmaster.

There are several reasons
1. When you're given a loaded gun, you should always check it. If you're an actor on a set with real guns, the production should give you at least rudimentary training.
2. There is little reason for live rounds to be used, and the armourer should make sure there're no bore obstructions.
3. As the producer Baldwin is responsible for the lapses in the above two.
4. Baldwin has been very vocally anti-gun in the past. To many people on the internet, this is the most important reason.
