In the Wizard of Oz, the first Tin Man inhaled aluminium powder from his face makeup, which triggered lung issues and he dropped out to get medical treatment, being barely able to breath. He survived and recovered though.
For his replacement, they used different makeup, but he struggled with it as well, among other things got an eye infection and almost lost sight on one eye.

Quite many, some died, some crippled for life. Shit ton of injuries.

they actually never even had to go to the hospital, miraculously it only shaved millimeters of hair off the lead actors head, they were totally fine. footage ends before we see them swim out from under the wreckage. However, pilot of the helicopter sustained a stubbed toe from the force of the crash.

A fire pole is a lot shorter, they don't leap on it, they don't go through wires on their way down, and at the bottom they don't drop on their back for the last few meters.

If I remember correctly the actor died, little vietcong boy got decapitated and viet girl got her arm cut off but survived.

I remember there being a guy on a bike taking a huge tumble that resulted in severe injuries as well, give some Aussie "stunt-pros" enough Fosters and you get movie gold.

The Resident Evil one where that chick had her arm ripped off and they didn't even want to cover her hospital bills.

In a transformers movie apparently during a car chase a safty wire snapped and left the stunt driver paralyzed and with brain damage, however they didnt used the take in wich it happens, thats why they had the re-use footage from The Island instead of the take.
