The harem is a marketing strategy, not an in-story reality. Most isekai protags don't even sleep with their main girl during the story, much less any of the rest of the female cast. There are few isekai protags that actually have a real harem as opposed to simply hanging out with a bunch of girls they are not sleeping with.

Sakura was supposedly based on the creator's wife, so it wouldn't surprise me.

The only isekai I can think of where the harem is an actual harem are Parallel Paradise and Creature Girls, which is why those manga are literally like 60% sex scenes and orgies.

In the first IDW Transformers run, Chromedome and Rewind are married. Chromedome has an extensive past relationship count - because his husbands kept dying. Rewind had one previous relationship, who was missing and presumed dead.
Rewind's reason for joining the comic's space adventures was in the hope that he might stumble across any information about his previous husband, having exhausted all other investigative avenues. Chromedome joined out of his love for Rewind.
The pair actually did find Rewind's previous husband alive, but severally brain damaged. Chromedome started to risk his own life in a procedure that might bring back the ex to full functionality, but Rewind forced him to stop, not wanting to risk his current love for someone that was already essentially dead.
(Also during the adventure Rewind joined Chromedome's list of dead husbands, but was replaced by a duplicate created by a quantum warp engine accident. Comics!)

The same comic run had several other committed, loving relationships.

Sakura is a foil. She rejects the protagonist, so she's punished by being stuck with a deadbeat dad. Immoral whore contrasted with ideal yamato nadeshiko, as you almost always see in manga. You can't feel truly superior and secure in your tastes without a Sakura to your Hinata.
