The film has one gory scene in the beginning but the guy doesn't die (he gets tortured by Rambo).
I guess what triggers them is that women get enslaved and the Cartel is shown as evil. There isn't even racial commentary at all and Rambo and his niece only walk in a very small microcosmos. The movie literally takes place only on the farm and one single block in Mexico. That's like ghettos with a certain kind of people doesn't exist according to them.

People are upset because it depicts real-world cartel members (who are ethnically Mexican) as bad guys, instead of depicting fantasy cartel members (who are white) as bad guys.

James Cameron's original script for Rambo 2 was full of proto Marvel soi quips. Rambo also would've had a "funny" sidekick constantly making bad jokes and acting like a basedboi. You can read the whole script on his website.

I only know that Camerons Rambo 2 would have taken place in a mental asylum and was supposed to be like the first one.

Apparently, according to Stallone, First Blood was originally a lot like that too. He had tons of dialogue, making little jokes here and there, and one point referring to an owl he was hunting in the woods as a 'mouse-munching piece of trash' or something like that. Obviously all that got left on the cutting room floor and they made it so he was more the silent determined type which was better. I think this was explained by him in a Stern interview.

I think he says that he wasn't even going to release it at all at one point.
